What To Expect
Please arrive 15-30 minutes prior to your appointment.
You will check in with our admissions team on your first visit. Please have your identification (Driver’s license or ID card) and a current copy of your insurance card. The admission staff will direct to the Outpatient Department. After that you don’t have to check in at admissions. You can always stop and say hello. You go right to Physical Therapy. Please wear comfortable clothes and shoes.
Also, please bring any paperwork with you that you have from your provider or surgeon. If you received a packet from Physical Therapy in the mail, please fill it out completely prior to your initial appointment and bring it with you.

We check and verify with your insurance that therapy is covered prior to your first visit. We do see worker’s compensation injuries as well!
You will check in at the hospital admissions desk located at the main entrance of the hospital. The front desk can then direct you to where the therapy clinic is located. For all remaining therapy appointments, you can directly check in at the front window of the therapy department.
We have paperwork that you will need to fill out prior to your first visit. We will try to mail this to you ahead of time so that you can bring it with you on your first visit. If you do not receive it in the mail, we will have you fill it out upon arrival on your first visit.
The therapist will conduct an examination which will consist of the following: your medical history, your current problems/complaints, how this problem affects your daily activities, your goals with therapy, medications and tests related to your health, and thorough testing to determine the extent of your injury. The therapist will discuss how therapy will address your problems and devise a plan with your input, including how many times you will see the therapist per week, how many weeks you will need therapy, home exercise program, patient education, short/long-term goals, and what is expected after discharge from therapy. This plan is created with input from you, your therapist, and your doctor.
You should wear loose-fitting clothing to expose the area that we will be evaluating and treating. For example, if you have a knee problem, it is best to wear shorts. If you have a shoulder problem, a tank top is a good choice. For low back problems, wear a loose-fitting shirt and pants so we can perform a thorough examination.
On average, each therapy appointment is approximately 45 minutes, but can sometimes take up to 1 hour.
It is possible that you may see more than 1 therapist during your plan of care to ensure we can treat you as best convenient for your schedule. Our team utilizes a collaborative approach and communicates consistently about your treatment plan of care. It can also be a benefit to have multiple therapists seeing you, as they can bring in new ideas for your treatment.
