It says so right in our name—Community Medical Center. We work with you so you will live your best, most productive life. CMC Community Outreach Services is an essential tool to keep our community healthy.
Chronic Care Management – Our program helps you manage your chronic conditions to help you live better. Our coordinator works with you and your provider to make sure medications are on track, regular check-ups are scheduled, and helps coordinate all the medical gobbledy-guck you have to wade through.
Diabetic Education – Our certified diabetic education program, taught by a skilled healthcare team of registered dietitians and a registered nurse, covers what you need to know about managing your diabetes so you can live a healthier life.
Good Beginnings – This is a voluntary home visitation program designated to promote healthy families and healthy children.
Healthcare Ambassadors – If you have a healthcare topic you may be interested in for a club or group meeting, please contact our Administration office or call 402-245-6503 for more information. Please note, we will try our best to meet requests but understand, sometimes it may not be possible with short notice requests.
Basic Life Support Class (BLS) – BLS Classes are held monthly. Call Brooke McCullough at CMC, 402-245-6659 to receive a copy of the schedule and register for a class.
