Community Medical Center, Falls City has a full-time opening for a Social Services and Discharge Planning Coordinator. This position will be responsible organizing services across provider lines, between people, agencies, and systems to affect optimal patient outcomes, and achieve continuity of care. This position also coordinates and facilitates discharge planning in collaboration with an interdisciplinary team to ensure a smooth transition from hospitalization to community-based care. The incumbent will serve as a point of contact for patients and providers as needed to ensure appropriate and timely access to care and services. This position will work with Utilization Review to determine appropriateness of admission, level of care, and continued stay. This position will also perform nursing duties as needed. Essential qualifications are a NE RN license, Bachelor of Nursing preferred. Management experience preferred, with the exceptional ability to be self-directed and highly organized.
