Community Medical Center, Falls City has a full-time opening for a Community Health Worker. This position will be responsible for helping patients to navigate and access community services and other resources. The CHW supports providers and care team through an integrated approach to care management and community outreach. The CHW must be knowledgeable about community resources appropriate to the needs of the patients and family. Essential qualifications are a high school diploma or GED, effective and proven communication skills, compassionate, empathetic, problem-solving skills and the ability to multi-task. A bachelor’s degree in related healthcare field is preferred. This position will be required to complete the Community Health Worker certification within the first 12 months of employment.
