With MyChart, you can request or grant access to view records of your children, spouse, parent or another adult. This is called proxy access.

Four Types of Proxy Access

You can request or grant access as a:

• Parent of a minor child
• Legal guardian of a minor child
• Legal guardian or personal representative of an adult
• Adult granting access to another adult

How to Request Proxy Access

This will be for CMC records but this process will go through BryanHealth. CMC is partnering with BryanHealth’s Epic Community Connect.

• Select the access you are requesting (below) and complete the form
• Send completed form, and any required documentation, to Bryan Health:

  • You can also drop the form off at your doctor’s office or the Health Information Management office at Bryan Health hospitals
• You can expect access within 24-48 business hours after the time we receive your form and any required documentation
  • If you have a MyChart account, you will see this person in a new ‘circle’ at the top of your MyChart account on your computer or tablet; if you are using the MyChart App, the ‘circle’ will be at the bottom of the screen
  • If you don’t have a MyChart account, you will receive an email with information on how to open/activate your account.
  • If you did not provide an email address, you will receive a letter with the information you need to set up your MyChart account
  • If you do not receive a response with 48 business hours, please call 402-481-8066 or email mychart@bryanhealth.org
