MyChart is a convenient, easy way for you to see your health records, test results and more for yourself and your family. You can manage appointments, request medication refills, communicate with your provider, pay bills and much more. Community Medical Center is partnering with BryanHealth to provide our patients with Epic MyChart.  When you need technical assistance you will reach BryanHealth’s technical support.

MyChart is a convenient, secure way for you to view your test results and health information online. You can also request appointments and prescription refills, communicate with your doctor, pay your bill and much more! With MyChart, you have one online resource where you can easily see your health information for care received at Community Medical Center and Family Medicine Clinics in Falls City and Humboldt.

With MyChart, you can manage your health any time of day, from anywhere using your computer or the MyChart app on your phone or tablet. Here are some of the many ways you can use MyChart to make your life easier:

    • View test results and health information
    • Set up and manage appointments (schedule, confirm, cancel and review details of upcoming visits)
    • Request prescription refills
    • Email your doctor, or any member of your Bryan Health care team
    • Access trusted health information resources
    • Manage your family’s health (yourself, your kids, etc.)

Is there a fee to use MyChart? 

No, it is free service offered to patients of Community Medical Center and Family Medicine Clinics in Falls City and Humboldt

What do I need to use MyChart? 

You need access to a computer connected to the Internet and an up-to-date browser such as: Internet Explorer, Chrome, Firefox or Safari.

You can also download the MyChart app for easy access to your health information anywhere on your phone or tablet

Is MyChart Secure?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames and passwords. Each person controls their own password, and the account cannot be accessed without that password. Unlike conventional email, MyChart messaging can only be done while you are securely logged into your MyChart account. No information crosses over the internet.

 

 

Enrollment Questions

How can I sign up?

You can sign up anytime, or we will provide information to you when you receive care at any Bryan Health location. Sign up is easy. Just answer a few questions and you’ll receive an activation code. Use your activation code to set up your username and password for your MyChart account.

Can you send me a new validation code if I lost it, let it expire or didn’t receive it?

Yes. Email us at mychart@bryanhealth.org or call us at 1-844-628-0303. After we verify your information, a new code will be sent via email to the address we have on file for you.

Community Medical Center is partnering with BryanHealth to provider Epic MyChart . BryanHealth’s technical team will be the ones who will be assisting you 24/7.

My activation code doesn’t work, what should I do?

For your security, your activation code will expire after 30 days and is no longer valid after the first time you use it. If your activation code is not working, you may email us at mychart@bryanhealth.org or call our MyChart Service Desk at 1-844-628-0303.

 

Technical Questions

I forgot my username, what should I do? 

From the MyChart sign in page, click the “Forgot password?” link. Then enter your MyChart Username, the last four digits of your Social Security Number, your date of birth and answer the security question to reset your password. If you are unable to change your password after following these steps, please contact our MyChart Service Desk at 1-844-628-0303 to request a new, secure password.

I didn’t receive a two-step verification, what should I do? 

If you have multiple email accounts, check one of the others to see if the verification code was sent there instead. If none of your accounts received the email, it could be that we don’t have an email address on file. If this is the case, please email us at mychart@bryanhealth.org or you can call our MyChart Service Desk at 1-844-628-0303.

I was logged out of MyChart. What happened?

We aim to protect your privacy and the security of your information. If there is no activity for 15 minutes while you are logged in, you will be automatically logged out of MyChart. If you need to leave your computer for any reason, we recommend that you log out of your MyChart session.

I am locked out of my account. What do I do? 

Please email us at mychart@bryanhealth.org or you can call our MyChart Service Desk at 1-844-628-0303.

I have multiple MyChart accounts. Can I link them together? 

Yes. To access your Bryan Health records, you need a Bryan Health MyChart account. The good news is you can link your MyChart accounts so that you only have to log in once, and you’ll have access to your health records in all your MyChart accounts.

How do I delete MyChart Account?

In order to delete your MyChart account, you will need to contact support. Please email us at mychart@bryanhealth.org or you can call our MyChart Service Desk at 1-844-628-0303
